Macular Degeneration

The macula is the central and most sensitive area of the part of the eye that “sees.” In a healthy eye, the macula records detailed images and sends them up the optic nerve to the brain, so they can be recorded as “sight.” As the macula deteriorates, the quality of these images is affected. At first, there may be little to no change in sight. But as the disease progresses, central vision may become blurry or wavy, and eventually central vision may be completely lost, leaving the patient with only their peripheral vision. At these advanced stages, patients may be considered legally blind.

Types of Macular Degeneration

There are two basic types of macular degeneration: “wet,” or exudative, which affects approximately 10-15% of patients, and the far more common type which is “dry,” or atrophic, which affects 85-90%.

There is also a third type which is much less common and occurs in children, called Stargardt disease.

Wet (exudative) Macular Degeneration

In this type, abnormal blood vessels known as CNV (choroidal neovascularization) grow under the macula and retina. After a while, the new blood vessels can bleed or leak fluid, causing a bulge which lifts the macula from its normally flat position. For people with “wet” macular degeneration, this is the cause of their distorted or lost central vision. In these cases, vision loss can be rapid and severe.

Dry (atrophic) Macular Degeneration

In this type, small yellow deposits known as drusen form under the macula. This leads to a thinning and drying of the macula in the areas where the drusen are present. This causes the macula to lose its function, and the patient to lose his or her central vision.

An interesting fact: nearly all people over the age of 50 have at least one small drusen in one or both eyes. However, only eyes with large drusen are at risk for “dry” macular degeneration.

This type of macular degeneration is much more common than the “wet” type, and progresses much more slowly. In some cases, people with the “dry” type can develop the “wet” type over time.

Stargardt Disease

This type of macular degeneration is passed down genetically from the parents. It develops in children ages 6-20. “Stargardt Disease is the result of a gene called ABCA4 and is usually a recessive trait. When both parents carry the ABCA4 mutation, there is a 25 percent chance their children will have Stargardt disease.”

What happens in Stargardt disease is that flecks of waste material build up in the nutritional support layer (RPE) for the rods and cones of the retina, causing all three to break down and impair vision.

Stages of Macular Degeneration

There are three stages of age-related macular degeneration (AMD).

Early AMD

Most patients do not experience vision loss at this stage. This highlights the need for regular eye exams, so that onset can be detected by your ophthalmologist. In the early stage, AMD is diagnosed by the presence of medium-sized drusen (yellow deposits beneath the retina.)

Intermediate AMD

At this stage there may be some vision loss in some patients; others still may not notice any symptoms. A comprehensive eye exam with specific tests will look for larger drusen and/or pigment changes.

Late AMD

Vision becomes noticeably impaired at this stage.

Macular Degeneration Risk Factors

The most important risk factors for macular degeneration are:

Age

The largest risk factor for AMD is age. Those who are 55 or older are most likely to experience AMD, and your risk factor increases as you age.

Genetics

If you have a family history of AMD, you are at a higher risk. For Stargardt disease, you are at risk if both your parents carry a recessive gene.

Race

Caucasians are more likely to develop AMD than African-Americans and Latinos.

Smoking

Smoking Cigarettes doubles (!) your risk of developing AMD.

Treatment

There is currently no known cure for AMD, but there are things you can do to keep your eyes as healthy as possible. Staying healthy will reduce your risk of developing AMD and might even slow the disease’s progression once it has developed. Don’t smoke, get enough exercise, eat healthy, get enough sleep, and wear sunglasses to protect your eyes from ultraviolet light.

Work Those Muscles

Eye exercises are designed to strengthen your eye muscles, help you focus and stimulate your brain’s vision center. As with any muscle, it only gets stronger if you work it out! As you continue to do these exercises, you will begin to develop more control over your eye muscles which can improve your vision.

Exercises are unique to each individual and can also be based on age and specific eye problems. These are some very simple techniques you can do to exercise your eyes at home:

• Change your focus from an object that is close to you to an object further away and back again.

• Cover one eye and focus on different objects.

• Concentrate on looking at a solitary object.

• Follow a pattern to build your eye muscles.

Additional Exercises

There are many other exercises you can try at home to help strengthen your eyes and combat eye strain. Try one or a combination of all six to develop a routine that can help you work those eye muscles.

1. Try rubbing your palms together to generate some heat and then putting your hands over your closed eyes and applying some gentle pressure. Allow the heat from your palms to warm your eyes and relax them for a few minutes.

2. Computer-induced eye strain contributes to a lack of blinking which can dry out your eyes. A simple exercise goal is to try and blink once every four seconds to keep your eyes lubricated. Sounds simple but when you are engrossed in computer tasks, it is easy to forget.

3. When you feel your shoulders tightening up and your neck starting to ache, you probably start some relaxing neck rolls to loosen up. You can do the same for your eyes. Close your eyes and roll them in circles. This will help keep your eyes well lubricated and can ease the strain.

4. In addition to the exercise mentioned above where you adjust your focus from an object near and then one far, you can also practice visual scanning. Focus on one or two items to start and then take in everything in the room around you, even looking out the window if you can. This allows your eyes to be in a constant state of motion.

5. It’s also important to take a break from that computer screen and focus on something, anything, else for a few seconds or better yet, a few minutes if you can. You should try to do this at least once an hour.

6. Another exercise to help avoid strain and strengthen those eye muscles is to practice the art of glancing. Sit down and close your eyes. Glance as far up as you possibly can and hold your head there for a moment. After a few seconds, look down and repeat this process for about 30 seconds. Now, with your eyes still closed, do this again only look to the right and then to the left. Continue to repeat for about 30 seconds.

Double vision is caused by a number of things, some which are treatable and not too concerning, while others may be more serious.  Below are the most common causes of double vision.

Squinting.  A condition known as squinting is when the eyes are not properly aligned.  It is a condition more common during childhood and causes the eyes to look in different directions.  The eye muscles can become paralyzed, weak or have restricted movement.  In some cases, patients have experienced double vision following treatment for squinting.  This is typically the result of the brain having to work so hard to send signals to maintain “normal vision” for so long that it takes some time to adjust.

Stroke. When a patient experiences a stroke, it is not uncommon for them to also experience double vision.  The blood vessels that supply the nerves that control the eye muscles can be impacted during the stroke and patients can have difficulty seeing clearly.

Thyroid. The thyroid has been known to affect the muscles that control the eye.  For those that suffer from Grave’s disease (also known as hyperthyroidism), patients can experience double vision as a symptom.

Keratoconus.  Keratoconus is a degenerative eye condition that causes the cornea to thin and begin to form the shape of a cone.  This causes the clear tissue in the front of the eye to bulge outward.  This condition can impair the vision.

Multiple Sclerosis.  Multiple sclerosis (or “MS”) can affect the nerves in the spinal cord or the brain, which may also include the nerves that control the eyes.  If this happens, one can sometimes experience double vision.

Brain Tumor or Cancer.  A tumor or growth behind the eye can damage the nerves around the eye and even prohibit movement of the eye.  This can cause double vision while the eye is unable to freely move and function properly.

Migraine Headaches.  For those that suffer from migraine headaches, a severe and often chronic form of headache that begins to take on other physical ailments, it is not uncommon to experience double vision.

Dry Eyes. When the eyes do not produce enough tears, they begin to dry out.  If they become too dry, they can become inflamed and sore.  This irritation from dry eye syndrome can result in double vision.  For those that experience chronic dry eyes, a prescription for lubricating and hydrating the eyes may help provide the eyes relief.

Temporary Double Vision

There are times when double vision is temporary. This can be anything from alcohol intoxication, seizure or epilepsy medication, or even head injuries. Head injuries are a common cause of temporary double vision. Even though the double vision may be temporary, it can still be a symptom or sign of a much more serious condition. Additionally, if you experience double vision following a head injury, it is important to seek medical treatment. Concussions can seem minor at first, but can become very serious very quickly and should not be taken lightly.

As one can see from the variety of potential causes of double vision, some may be minor or temporary and not require much care. Google Plans to Diagnose Eye Diseases with Artificial Intelligence

The healthcare industry continues to grow and advance with the development of technology and innovation. How Does Google Plan to Use DeepMind Health in Eye Care?

One way doctors diagnose signs of eye diseases is by examining the interior of the eye. They can do this either directly, with an ophthalmoscopy or by taking a digital optical scan. Another technique is to take a non-invasive three-dimensional scan of the retina using a process called coherence tomography. These methods are labor intensive. In efforts to help with the time-consuming process of analyzing the scans, DeepMind Health is utilizing their technology to speed up diagnosis and treat more patients.

DeepMind Health is starting a new research project to help doctors spot the early signs of eye diseases. This new project, which is based out of U.K.’s Moorfields Eye Hospital in East London, is the second partnership with U.K.’s NHS team. The company plans to use their machine learning device to analyze more than one million anonymous eye scans, creating algorithms that can detect early warning signs that current technologies and humans might overlook.

More importantly, DeepMind Health is hoping to spot two eye conditions in particular: wet age-related macular degeneration and diabetic retinopathy.

While the project is in its infancy, artificial neural networks like the products of DeepMind’s research have already shown great promise in the field of healthcare.

What Controversies Has Google Stirred Up with DeepMind Research?

Back in February, DeepMind Health worked with NHS to develop patient care software, which stirred up controversy. One app was Streams, which gave doctors information about their acute kidney failure patients, and the other was Hark, which helps doctors and nurses organize information that is currently managed with hand-written notes.

The project was scrutinized, in which DeepMind Health was granted access to full medical history of 1.6 million NHS patients who were not made aware of that fact. This data included records from London’s Royal Free Hospital, Chase Farm and Barnet hospitals over the past five years and until 2017. As a result of this, some privacy advocates protested. The Information Commissioner’s Office, the U.K.’s data protection department, arranged investigations in May.

The new project seems unlikely to provoke similar concerns and outrage, since the data is completely anonymous.

Conclusions

Two million people suffer from vision loss in the U.K., of whom around 360,000 are registered as blind or partially sighted. With the right treatment in a timely response, many cases are preventable.

With the number of people with vision loss in the U.K. predicted to double by 2050, Moorfields Eye Hospital and NHS Foundation will partner with DeepMind Health to explore how artificial intelligence can help medical research in diagnosing eye diseases. The history of optometry is rich and full of events, people, organizations, and stories. Many significant histories document, in detail, the development of optometry and ophthalmology, and how it has led to today’s eye care advancements.

Early Modern Optometry: Foundations

One of the first figures associated with the invention of spectacles was the thirteenth century English philosopher, friar Roger Bacon. In 1263, Roger Bacon realized the potential of using lenses for those weak-of-sight. In his Opus Majus, he wrote, “If anyone examine letters or other minute objects through the medium of crystal or glass…he will see the letters far better and they will seem larger to him.”

By 1286, the first pair of spectacles were invented by an unknown party. By 1300, Europeans fashioned spectacles and often referenced German mathematician, Johannes Kepler, for his pioneering work on the function of the eye. He proposed that images are projected inverted and reverse by the eye’s lens onto the retina. Kepler’s work sparked a series of development in optometry, and in 1629 England granted its first charter to spectacle makers.

The Era of Progression

In 1799, Scotsman John McAllister Sr. opened the first U.S. shop for optometric services. By the end of the 18th century, Benjamin Franklin had invented the bifocal lens (1784), John Dalton brought colorblindness to public discussion (1794), and Thomas Young successfully mapped the normal visual field (1801).

In 1856, Hermann von Helmholtz published his handbook of Physiological Optics. In 1865, cylindrical lenses began being used and optometry as a term began being used in the practice.

In 1888, the first successful use of corrective vision contact lenses was documented. That same year, William Bohne’s Handbook for Opticians was published.

By 1921, the United States recognized optometry as a profession, which helped solidify its importance in the greater medical field. From the mid-1950s and through to this day, optometrists have developed a variety of new technologies and methods to examine the eye and improve vision – these include advanced contact lenses, laser surgery, LASIK, and more.

The Age of Technology

Auto-Refractor Keratometer measures an objective refraction or theoretical refraction of what lens power your need for proper vision. Non-Contact Tonometer measures intraocular pressure, which is the pressure inside your eyes. This instrument calculates and converts the response into a measurement of pressure.

Tonopen also measures intraocular pressure. The device touches an anesthetized eye surface to measure pressure and is most commonly used for patients with limited mobility.

Visual Field Analyzer, also known as a peripheral or side vision test, is used for glaucoma patients and patients suffering from headaches, visual disturbances, decreased vision, blind spots, and systemic conditions.

Pachometer measures the thickness of the cornea of your eye. This instrument helps determine correction factors for pressure measurements.
